Introduction: PCNL(Percutaneous Nephrolithotomy) is the treatment of choice of staghorn renal stones; however this procedure has its own complications including acute kidney injury. Due to rapid and accurate management of this complication we should diagnose it as fast as possible. There are lots of renal function markers for that purpose. The aim of this study is to evaluate renal function before and after PCNL by urinary NGAL measurement. Methods: this is a cross-sectional study on 41 staghorn renal stone cases aged 20-70 year old. After taking consent from patients they entered the study. Demographic and surgery related data documented. NGAL levels measured by urinary sampling, 2 hours before and 12 hours after the surgery and serum Cr was measured 12 hours before and 48 hours after the surgery. Finally the collected data was analysed by SPSS software. Results: there was 29 men and 12 female in 41 participants of this study. Most of the patients were older than 55 years old. Serum Cr showed an increase after the surgery and GFR decreased. NGAL levels showed a significant increase in urine 12 hours after the surgery.Discussion: There are a lots of markers to evaluate renal function before and after the surgery. The most common marker is serum Cr, but because of its weaknesses, we used urinary NGAL as a better marker of acute renal damage. NGAL showed a bigger change before and after the surgery in comparison with serum Cr. it possibly show the accuracy of NGAL to detect small changes in acute phase of kidney injury
