Abstract : Background: oral contraceptives (OCP) have been previously reported to be a risk factor of venous thromboembolism and pulmonary embolism. But regarding previous studies, its effect on cardiovascular disease (CVD) and stroke is still controversial. In this study we aimed to clarify if there is an increased risk of future CVD in women with history of using OCP.Methods and material:This cohort study was conducted during years 2001-2011 in a group of women aged 35 or more. All participants were divided in two groups with history of OCP using and non-using. A questionnaire containing demographic data, history of using OCP and other risk factors of CVD was fulfilled. Body mass index, hypertension and blood's biochemistry markers including fasting plasma glucose, total cholesterol, triglyceride, high density lipoprotein and low density lipoprotein were evaluated at the beginning of the study. Stroke, Myocardial infarction (MI), sudden cardiac death and total CVD were assessed during the study. At the end, all gathered data were analyzed using SPSS-15 software; Chi-squared and independent T-test were used for comparing group and for evaluate association between CVD event and using OCP cox regression model were used. Results: Out of 3254 women aged 35 or more participated in this study, 1391 individuals (42.7%) had history of OCP using and 1863 individuals (57.3%) had no history of OCP using. There were differences between groups (OCP users and non-users) about age (P-value≤0.001), Hypertension (P-value≤0.001) and waist circumference (P-value=0.009). But there were no differences in the field of diabetes mellitus (P-value=0.353), Fasting plasma glucose (P-value=0.177) and dyslipidemia (P-value=0.368). None of the events including MI;HR: 0.514 (0.288-0.919), stroke ;HR:0.803 (0.501-1.287), sudden cardiac death;HR:0.39 (0.156-0.97) and CVD events;HR:0.802 (0.642-1.003) showed a significant relationship between Event and using OCP by comparison of those OCP users and non-users even After Adjusting for demographic data and risk factors the same results were seen.Conclusion: By contrast to previous studies, our data revealed no increased risk of future stroke and CVD events including MI, stroke and sudden cardiac death by history of using OCP. History of using OCP for a longer period of time compared with lesser period of time showed no difference about prevalence offuture CVD.
