Abstract

Introduction: Aortic root stiffness is a high prevalent condition in patients with chronic kidney disease (CKD) which independently associate with increase risk of mortality and morbidity in this group. The purpose of this study is to investigate the relation between levels of GFR as a marker of renal function and elastic properties of aortic root obtain with echocardiography.Methods and materials: 82 patients (53 male and 29 female with mean age of 57.9±15.0 years) with CKD stage 3and 4 lasting at list for 3 month (GFR 60 ml /min/1.73m2) or lower were enrolled in this study. Echocardiography and tissue Doppler imaging of upper and lower aortic walls was done. Systolic wave (S wave) obtained from each walls. Aortic distensibility and aortic root stiffness index calculated using blood pressure measurement and aortic dimentions in systole and diastole by M-mode echocardiography. Results: In all patients aortic stiffness index (14.24±9.82), aortic distensibility (0.018±0.015), aortic upper wall S velocity (0.10±0.03) and aortic lower wall S velocity (0.08±0.02) were severely impaired. There was no correlation between GFR and aortic distensibility (r=-0.253, p=0.198), aortic root stiffness index (r=0.193, p=0.086), aortic upper wall S velocity (r=-0.106, p=0.170) and aortic lower wall S velocity (r=-0.150, p=0.09). Conclusion: Aortic stiffness was seen in all patients with renal failure in stage 3 or 4 but we could not find any direct relation between GFR and this phenomenon. So aortic root stiffness may be a consequent of hypertension and other risk factors
