Abstract

Background: Recurrent vulvovaginal candidiasis (RVVC) is a common cause of morbidity in women affecting millions of women worldwide. Patients with RVVC are thought to have an underlying immunologic defect. This study has been established to evaluate cell mediated immunity defect in response to candida antigen in RVVC cases.Methods: T-cell response to candida Ag was assessed in 24 women with RVVC, 7 known cases of chronic mucocutaneous candidiasis (CMC) and 20 healthy women.Results: T lymphocyte proliferation in response to candida was significantly lower in cases and CMC patients compared to control individuals (P-value< 0.001), but no statistically difference was seen between cases and patients with CMC (p-value>0.05).Family history of primary immunodeficiency disease differ significantly between groups and was observed in 42.9%, 29.2% and 0 % of CMC patients, RVVC cases and controls respectively (p =0.008). Prevalence of atopy was greater in cases compared to healthy individuals (37.5 vs. 10%,p =0.03). Patients with history of allergy showed higher lymphoproliferative activity and milder clinical symptoms in comparison to patients without allergy (p=0.01, p=0.02). Conclusion: Our findings revealed that patients with RVVC might have either very low or near normal T cell proliferation in response to candida Ag. Cell-mediated immunity defect is the underlying cause of vulvovaginitis in cases with impaired T cell response and allergic response in mucocutaneous membranes might be responsible for clinical vaginal symptoms in RVVC patients who had history of allergy simultaneously
