Abstract

Introduction: Premature rupture of membranes (PROM) is one of the most common complications of pregnancy which has a major impact on pregnancy outcomes. A diagnostic tool that is non-invasive and specific is needed to predict PROM. So the purpose of this study was to evaluate the diagnostic power of prolactin level in vaginal discharge for diagnosis of premature rupture of membranes and to determine cut-off values.Material and Methods: The 80 pregnant women were recruited in this diagnostic trial. The PROM group comprised pregnant women visiting the hospital between 20 and 37 weeks of pregnancy to check for amniotic fluid leakage and receive a confirmed clinical diagnosis of premature rupture of membranes through sterile speculum examination. The Control group consisted of women with normal pregnancy or control visiting the hospital during 20 to 37 weeks for reasons unrelated to amniotic fluid leakage and showed no evidence of leakage of amniotic fluid through the sterile speculum examination. All patients underwent speculum examination for amniotic fluid pooling, nitrazine paper test, fern test, vaginal washing fluid prolactin sampling. Finally; the collected data were analyzed by several software including Med.cale and SPSS(ver.20), and T-test, chi-square, and ROC.Result:Level of prolactin was significantly differently between the two groups (173.41±41.41ng/ml and 1.68±0.91ng/ml, P value <0.0001). The sensitivity, specificity, positive and negative predictive values were 98.80%, 100.0%, 99.9%, 89.9in detecting PROM by evaluation of vaginal fluid prolactin concentration with a cut-off value of 3.4ng/ml, respectively.Conclusion:The prolactin Levels, in comparing with other tests such as Ferning and Nitrazine and amniotic rate, was an appropriate diagnostic measure with high accuracy and sensitivity to detect PROM.
