Abstract : Percutaneous nephrolithotomy (PCNL) is the standard treatment for kidney stones larger than 2 cm in size. It has its own complications as any other invasive procedure. Acute Kidney Injury (AKI) is a very rare but serious complication of PCNL. The aim of this study was to determine AKI using Cystatin C as the marker of kidney injury.After taking consent from candidate patients, PCNL surgery was done. Their demographic data was collected. Serum Cystatin C levels was measured 6 hours before and 12 hours after the surgery and analyzed using Kolmogorov-Smirnov test and Paired T test.33 patients including 12 females and 21males was participated in this study. The average age of participants was 42.7 years. The most common type of stone in these patients was calcium oxalate. Cystatin C and Creatinine (Cr) levels before and after the study didn’t show any significant difference (p Value > 0.05).This study showed that Cystatin C increases acutely after PCNL surgery but its change, is in normal range and due to its high specificity and sensitivity of for prediction of kidney injury, none of the patients presented with AKI as it was predicted by cystatin C levels before and after surgery.Implication for health policy/practice/research/medical education:This study aimed to determine Acute Kidney Injury (AKI) in patients that undergo PCNL with a better marker than creatinine for acute phase of the injury. First of all it showed that PCNL is a safe minimally invasive surgery even in the first hours after the surgery and the damage to the kidneys is not significant. And this study used the Cystatin C as a good marker for acute phase of damage witch is more accurate and reliable. But because of the low rate of AKI in PCNL in our samples it wasn’t occur in any of the participants. But in the end we can say that Cystatin C is good serum marker to determine kidney function in acute injuries.
