Abstract

Background: Differentiating malignant effusions from benign reactive effusions has great diagnostic and prognostic value and maybe very difficult in some cases. Several markers considered to be useful for this purpose, yet there is some controversy. We studied the value of D-dimer level in serous effusions for differentiating malignant effusion from reactive ones.Materials and methods: 72 adult patients with serous (pleural and peritoneal) effusion studied in this cross-sectional study from April 2014 to April 2015 in Alzahra hospital. Smears and cell blocks from samples prepared and reviewed and D-dimer level in fluids measured with immunometric flow-through method by Nycocard Reader. All cases divided to two groups: malignant effusion and benign (reactive) effusion. Parameters of diagnostic value of D-dimer level between two groups determined.Results: 72 patients with mean age of 59.9 years with pleural or peritoneal effusion were examined. 53 patients had benign reactive effusion and 19 patients had malignant effusion. Mean D-dimer level was 13.46 mg/l in malignant fluids and 8.45 mg/l in reactive fluids and this difference was statistically significant (p< 0.001). The cut-point of D-dimer level was 10. PPV and NPV were 2.2 % and 99.8 % respectively. Area under the curve of ROC was 0.81 (p<0.001). Conclusion: according to our findings, D-dimer level in malignant effusions was considerably higher than reactive effusions. Furthermore this test has high sensitivity and NPV for diagnosis the type of effusion but has low specificity and PPV. Regarding the accuracy of 68.06% this test can be helpful to differentiate malignant effusion from reactive effusion.
