Abstract

|
Objective: This study is aimed to evaluate the complications of coronary artery bypass graft (CABG) surgery in patients over 70 years old in comparison to younger patients.Methods: Patients undergoing isolated CABG were enrolled in this study and were divided in two age groups: Group 1 (age<70 years) and Group 2 (age 70 years). Medical records of all patients were reviewed and collected and frequencies of CABG complications were compared between these groups.Results: A total of 3036 patients (568 patients over 70 years old) were included in this study. Our results showed higher incidence of postoperative complications among older patients including multi-organ failure (0.9% vs. 0.1%, respectively; P=0.003), mild and significant pleural effusion (49.2% vs. 41.3% and 5.7% vs. 3.2%; P<0.001), transient and permanent stroke (1.1% vs. 0.2% and 1.2% vs. 0.4%; P=0.002) , in-hospital mortality (3.5% vs. 1.1%; P<0.001) and cardiac arrhythmia (27% vs. 11.5%; P<0.001). In addition, longer duration of hospitalization was seen in the older group (8.31 ± 4.78 vs. 7.43 ± 4.88 days; p<0.001). However, post-operative renal failure requiring dialysis incidence did not show significant difference between the studied groups (0.4% vs. 0.3%; P=0.586).Conclusion: This study indicates that patients over 70 years old are at increased risk of complications after CABG compared to the younger patients almost in all fields. Therefore, postoperative observation of older patients with precise monitoring is necessary to prevent these events.
