Abstract : Background: Current infertility treatment techniques mostly need hormonal induction by high doses of gonadotropin. Howeverall cases do not response similarly to gonadotropin.Previous studies have implied that administration of testosterone or DHEA may result in an increase expression of FSH receptors.So wedecided to study the effect of testosterone gel administration on follicles and ovaria response to FSH, in poor responder women.Methods:This randomized double-blind placebo-controlled study was done in infertility center of Shahid-Beheshtihospital from April 2014 to march 2015. 64 women with poor responses to usual fertility treatments enrolled in this study. Eligible participantsrandomly allocated into two groups, 28 patients to intervention and 28 to control group.Each individual was studied during two consecutive cycles. First cycle was regarded as control cycle and no additional intervention applied. Second cycle was intervention cycle, and patients in two groupswerereceived testosterone orplacebo gel. After the period of using the gel, patients were analyzed according to their antral follicle counts then long term ovarian stimulation protocol was applied for patients.Results:Ultimately, 49 patients were analyzed in this study. Antral follicle count at HCG stimulation day was higher in testosterone group for follicle size >14mm (p-value<0.001) and between 10-14mm (p-value=0.001). Serum testosterone levels in intervention group was increased significantly (p-value=0.003). The total oocyte retrieval was the same in two groups (p-value=0.80).Conclusions:This study implied that administration of testosterone gel did not improve the result of oocyte retrieval. Therefore this technique did not improve IVF/ICSL result
