Abstract

Background: Chronic obstructive pulmonary disease is one of the most common diseases in the society that besides causing respiratory problems, would lead to other complications such as increase in pulmonary arterial systolic blood pressure, reduction in lung vital capacity and vascular disorders including enlargement of right ventricle and atrium and valve problems in patients. On the other hand some studies have shown that using Spironolactone have been effective on improvement of the above mentioned indexes including reduction of pulmonary arterial systolic hypertension but the conducted studies in this field are limited; therefore the present study was conducted to evaluate the effect of Spironolactone on controlling pulmonary arterial systolic blood pressure in patients with stable chronic obstructive pulmonary disease who referred to Al-Zahra hospital during 2014.Methods: In this study 37 patients with chronic obstructive pulmonary disease were selected as the intervention group and received Spironolactone 25 mg every day and another 42 patients who were selected as the control group received placebo. All the patients at the beginning of the treatment and 6 months after that underwent echocardiography and spirometry and the changes in spirometry and echocardiography parameters of both groups were compared.Results: The mean of pulmonary arterial systolic blood pressure in the control and the intervention group before the intervention was 38.83 ± 6.9 and 39.65 ± 8 respectively and no significant difference was observed between both groups (p = 0.56). This mean score after the treatment in the intervention and the control group was 21.73 ± 6.12 and 24.5 ± 7.42 respectively but the difference between both groups was not statistically significant (p = 0.08). The mean of differences in the pulmonary arterial systolic blood pressure in the control group was 25.4 ± 5.2 and in the intervention group was 27.6 ± 5.2 and the difference between both groups was not significant. Also according to the results the improvement in spirometry and echocardiography parameters of both groups was not significant.Conclusions: According to the results of the present study using Spironolactone in patients with COPD had no significant effect on reduction of pulmonary arterial systolic blood pressure and spirometry and echocardiography parameters; so further studies are required on this matter.
