Abstract : Objectives:Transient ischemic attack (TIA) patients are at a high cardiovascular risk. This study aimed to evaluate diagnostic value of CIMT (carotid artery intima-media thickness) and ABCD2 score for predicting cardiovascular events in long-term follow up after transient ischemic attack.Patients & methods:60 patients with transient neurologic deficits who were admitted in the emergency ward, Alzahra hospital, Isfahan, Iran from March 2013 to August 2013 were studied prospectively. Duplex ultrasonography of both carotid arteries was performed on all subjects, usually on the day of presentation. ABCD2 score on admission was evaluated retrospectively. At a median follow-up of 20 months patient were asked about new vascular events. Data were analyzed using independent t test, chi-square test and one way ANOVA, correlation and receiver operating characteristic (ROC) curves.Results:The mean age of our 60 patients was 63.57(SD=12.01) years. There were 22(36.7%) females and 38(63.3%) males. ABCD 2 subgroups were distributed as follows: 0-3 points, 16 (26.7%) patients; 4-5 points, 31 (51.7%) patients; 6-7 points, 13 (21.7%) patients.Follow-up data were available for all sixty patients .In 12 (20%) patients an ischemic stroke and in 7 (11.7%) a new TIA was diagnosed. Four (6.7%) patients were diagnosed with acute MI; at the time of follow-up, 4 (6.7%) patients had died for the cardiovascular ischemic event. For 33(55%) reminding subject no cardiovascular ischemic event were happened within the follow up period after index TIA.The cut off enabling prediction of cardiovascular events was 0.855 mm for CIMT and 3 for ABCD2score.Sensitivity, specificity, and positive and negative predictive values for CIMT were 88.8%, 57.5%,63.1% and 86.3% (overall accuracy, 71.6%) (P=0.002 and Area under the Curve (AUC) =0.736, 95% CI, 0.611-0.862). In comparing with IMT, Sensitivity, specificity, and positive and negative predictive values for ABCD2 score were 85.1%, 36.3%,52.2%, and 75% (overall accuracy, 58.3%),[(P=0.063and Area Under the Curve(AUC)=0.640; 95% CI, 0.501-0.779)].Conclusion:Our study showed that, in comparing ABCD2 scores; CIMT was more accurate to predict cardiovascular event in long term follow up. Addition of CIMT measurement in the ABCD2 score may enable prediction of long-term vascular risk after TIA.
