Abstract

Introduction: Central venous catheterization (CVC) is a commonly performed procedure in critically ill patients, which needs accurate, safe, and more efficient method in order to confirm misplacements faster. Therefore the aim of this study was to compare ultrasound with saline flush method and CXR for confirmation of above-the-diaphragm CVC placement.Methods and materials: This prospective longitudinal cohort study was performed on 103 patients who had indication for CVC in emergency department. CVC placement was evaluated with saline flush method and CXR, then Computed Tomography (CT) was performed as gold standard. The outcomes, sensitivity, specificity and total confirmation time were compared in both groups.Results: The mean time for saline flush method in order to confirm the placement of CVC was 2.5±1.24 min (0.5-5 min) and in CXR method was 32.11±5.52 min (20 – 45 min) (P<0.001(. The specificity ,sensitivity, negative and positive predictive value, overall accuracy were 100%, 80%, 98.98, 100% and 99.02 %, respectively, while in CXR method were 100%, 60%, 98, 100% and 98.05%.Conclusion: Our results showed that confirmation of CVC placement by saline flush test is an accurate, safe, and more efficient method than confirmation by CXR. It allows the central line to be used immediately, expediting patient care and more critically ill patients may benefit from these advantages of this method.
