Introduction: Dual pathology (DP) is used for simultaneous presence of hippocampal sclerosis (HS) and any other pathology in the brain. Since DP is less studied before, we aimed to evaluate the frequency and characteristics of DP among drug resistant epileptic patients with HS.Methods: This is a cross-sectional study conducted during 2007-2015 in Kashani Comprehensive Epilepsy Center, Isfahan, Iran. Patients with diagnosis of drug resistant epilepsy and HS where enrolled in the study and demographic data, seizure semiology, EEG findings, and MRI findings were collected. We compared these variables between three groups of DP, unilateral HS, and bilateral HS.Results: Of all 201 cases, 28 patients (13.9%) had DP, 21 patients (10.4%) had bilateral HS and the rest had unilateral HS. The mean age (SD) in patients with DP was 29.2 (9.3) and 19 of them (70%) were male. Patients with DP had more EEG activities from two or more sites (28.6% compared to 5.3%, respectively; P-value<0.001). Also, CPS was a more common presentation in patients with unilateral HS (86.2% compared to 50%, respectively). Comparison of disease characteristics between DP and bilateral HS showed no difference (P>0.05).Conclusion: we found DP among 14% of our drug resistant epileptic patients with HS. DP patients mostly presented with CPS and had ictal and interictal EEG activates from one site in the brain. Gliosis was the most common pathology among them patients followed by cyst, atrophy, and tumor. Patients with DP experienced more focal seizures and had more ictal EEG activities from two or more sites in the brain compared to patients with unilateral HS, while, similar behavior was seen in patients with DP and bilateral HS.
