AIM: To assess long-term clinical results of laparoscopic Heller Myotomy (LHM) following Dor fundoplication versus those without fundoplication in patients with achalasia.METHODS: Patients with idiopathic achalasia undergone laparoscopic surgery in a tertiary hospital, Isfahan, Iran were reviewed. The first group were the patients which undergone LHM alone, and the second group included patients undergone LHM followed a Dor Fundoplication. Objectives followed up by sequential visits at clinic (before surgery, 1month after and 6month after) for evaluation and fill the Eckardt questionnaire. The Eckardt score was used to evaluate four metrics: dysphagia, weight loss, retrosternal chest pain and reflux. RESULTS: We identified a total of 67 patients with a mean age of 38±10.1 and whom 61.2% were male. Thirty five patients underwent LHM while 32 patients underwent LHM following fundoplication. Just before treatment, Patients had a total score of 7.74±2.41 in LHM group and 8.19±2.40 in the LHM following fundoplication group (Pvalue>0.05). During 6mo follow up, within each group, Eckardt scores showed statistically significant decrease (Pvalue of both groups were <0.05). Comparing between groups also showed significant different (3.23±2.32 vs. 2.09±1.97 in the LHM and LHM following fundoplication groups respectively with a Pvalue = 0.03). CONCLUSION: Patients with achalasia undergoing LHM following Dor fundoplication have better clinical status than those treated with LHM alone
